Root SD CARD Unexpectedly Unmounted

I have a rooted stock ROM phone. About 2 months ago I started receiving this error. It hasn't caused me any issues as far as my phone working or anything like that. At first I thought it might be my card so I placed it in my computer and the computer had no problems with the card I left it in the computer for a few days to make sure it didn't give me problems. I put it back in the phone and the next day I received the error message again. I have since just taken the card out.

I wanted to know if anyone else has had this issue? I'm not stressing it any as I rarely used the card to being with but it was nice to have it for when I need it. Just has me curious as to if this is a hardware problem with my slot? or if its a software problem
 
Strangely enough This started on the wifes phone 5 days after factory warranty ran out. Message says sd unwxpectdly removed. She took it to verizion, and of course they tried to sell her up. But they also told her to take it home remove card put in computer if you can read and access your stuff the card is good. The issue might be the sd card reader in the phone. I too am looking for guidence.

Anyone???
 
After a bit of thinking about it I decided to change roms and see if infact this is a rom issue. I'm going to give it a few days and see how it does. For those interested I put PA on here and so far its very nice.
 
I would at this point venture to say that it is most definitely an issue with the ROM. PA hasn't had any issues with the external sdcard no unexpected un mounts
