Upon flashing new recovery I noticed that it didn't work with flashify. It caused an error. I then used fastboot to flash the recovery and it works great so far. However, somewhere along the line I lost the ability to enter recovery using volume - and the power button. How can I go about getting it back?

twrp-3.0.0.0-20160206-w5.img
changelog - cm-13.0-20160214-NIGHTLY-w5.zip
BETA-SuperSU-v2.67-20160121175247.zip
open_gapps-arm-6.0-pico-20160214.zip

When executing each group of updates, I clear cache and dalvik before and after installation.
But at all times, at the beginning of each formatting or in attempt to repair file system an error occurs:

"Unable to recreate and-sec folder."

"Repairing Data using e2fsck...
Done.
Repairing Sandisk SD Card using fsck.f2fs...
/sbin/fsck.f2fs /dev/block/dm-0 process ended with ERROR: 255
Unable to repair Sandisk SD Card
"

How to fix this?

EDIT:
I made backup my data, formatted my sd card and deactivated adoptable storage.
