URL from "logcat_accept.txt" file! :)

I renamed the file, but after it is recognized by the system during the update it doesn't find the necessary HBOOT file. My theory is that, if anything, adding the HBOOT file from the other update might get it to start but there is no telling if anything else is missing.

this isn't a HBOOT type file, it's an upgrade. Not sure if we can actually use this to update... try naming it update.zip

Pretty good theory. I tried the same thing that you did with the same results. It is likely that the HBOOT file is not included in the OTA since it does an "in-place" upgrade.
 
So yeah, it is an in place upgrade that will likely not vary much from the v3 leak. Credit to Caddy and others that have already pointed this out ad nauseum.
 
I think a script from Verizon is loading the files from the ZIP, it's not self running. I don't think you can manually run it. There is a boot.img file in the zip though.

A previous post here, or on XDA, stated that even if we have this file, we won't be able to use to upgrade unless we have ROOT. The file needs to go into the /cache folder, which we don't have access to.

I was afraid of this. I figured it downloads it to internal storage and that's where the phone looks for updates.

Just a shot in the dark here:

Since VZW is claiming that this WILL overwrite the current V3 leak, I'm wondering if it skips the version check (or possibly even signature check) since it's coming directly from them. I'm guessing it's highly unlikely, but I kinda have my hopes up for a root exploit if someone figures out how to download the file and do a manual install.
 
Looking at the URL and thought some of the numbers looked rather familiar. Now, I know that there is a lot of talk/rumor that the v3 leak (which I currently run) would be the OTA.

Here are the software versions on the last two leaks:

Software number: 2.26.605.4 (Leak 2)
Software number: 2.36.605.1 (Leak 3)

Now, I am no software guy, by I do know when numbers match. That being said, my guess is that the OTA is a modified version of the v3 which wouldn't require a complete wipe of the phone (like the leak did).
