[Verizon] 4.0.4 Not Working Right! :(

I finally took the plunge and installed .4 today. I installed the official leak from Google so this isn't a custom rom or anything. I'm having some major issues though.

1. I'm not receiving any phone calls. If someone calls me it rings once or twice on their end and then goes to VM. I don't get any sort of notification during the call. It's only after the call has ended that I just get notification via my incoming call log. I don't even think I get a missed call notification. I have LOTS of people calling this phone since I run a business so I'm getting plenty of reports of this and I really need to get that fixed asap. I can make calls fine and I can make and receive texts fine.

2. I don't have a lock screen anymore. When I hit the power button my phone goes straight into it's last screen. I checked the settings to make sure I'm set to slide to unlock and I am.

3. My home button doesn't work...at all. I'm running Nova Launcher so maybe it's a Nova thing but I usually have my home key set to jump back to the home screen. It's not doing that. I tried setting it up again in Nova and it didn't work. I also tried assigning the home key different functions like open app drawer to see if that worked and it didn't.

4. My LightFlow notifications won't cut off. If I get a text I have LightFlow set to give me an orange LED notification. Normally I check my text and it goes away. It's not going away now. I just get persistent notifications.

Any ideas on these? ESPECIALLY number 1!
 
No. I'm reflashing it now using the full version deodexed version via Rom Manager. This time I selected to wipe dalvik cache. I didn't know you could wipe the other cache without actually wiping the phone. Am I mistaken there? I hope this works!
 
I'm running into exactly the same problems as the OP. Tried flashing the same Google update, had these problems, so I flashed again wiping both caches, which I thought I forgot to do the first time. Then I couldn't change the wallpaper and the Play Store wouldn't work, so I rebooted, and now I have these problems again.

GNex 4.0.4, rooted and unlocked, stock. Please help!
 
I too have the problem AGAIN.

I've been trying to get .4 to work for 12 straight hours now. Here's the latest, I tried installing a deodexed version of .4 on top of the patch and it didn't work. So then I broke down and bought a full version of TB and backed up all my apps. I didn't know if TB would save all my apps on my phone after a wipe so I attempted to grab the files from windows explorer but the phone wouldn't load right. Galaxy Nexus appeared but when I went into it, it was blank. I did several reboots and usb cable pulls to no avail. I saw that TB now has a Google Drive backup option so I did that...it took quite some time. Then to make sure my photos were protected I synced that with G+ since again I couldn't get into the folder to grab anything. Then I decided since I was doing all of this I would go ahead and flash a custom ROM. I had my eye on AOKP for months so I flashed the newest build that was released today. Everything worked great, the ROM was SUPER fast, etc but during my TB restore (didn't restore anything crazy) everything fell apart again. Play Store would open but wouldn't download anything, GApps were missing even though I flashed them several times, the home key stopped working AGAIN in both Nova and stock launcher and then phone calls stopped coming AGAIN. So now I'm wiping data and cache AGAIN and flashing AOKP AGAIN. I have no idea what's causing this but it's REALLY annoying! /rant
 
you need to wipe data and cache whenever you install something that is not the same rom as the developer makes it
IE:
Gummy can be flashed over Gummy but they suggest a data, and cache wipe
Gummy cannot be flashed over AOKP without wiping data and cache

so if you install a deodexed version of .4 you must WIPE DATA AND CACHE!

Regardless, I did wipe data and cache before installed AOKP and it still failed to work. I redid everything and I'm happy to say I'm up and running now and I must say, AOKP is everything and more that I hoped it would be. It's SUPER fast and the little mods and tweaks are a nice addition to ICS.
 
if you are applying something leaked to the original build of ICS that is not coming directly down from Verizon then the answer is yes, you should data wipe to avoid any real issues, the dirty flashes can cause some performance issues as well as framework FC's

Here's what I do everytime I flash a rom. Wipe data, wipe cache, wipe davlik, format system---then flash the rom.

When you restore with titanium, avoid restoring any system apps or data unless you know what you are doing there. Just restore user apps and data.

Other questions: what radios are you using and what gapps are you using?

Well now I'm running the new radios (4 and 5) and I'm using the newest version of gapps from AOKP. My signal strength isn't very good (~108) but I can't tell during phone calls and I don't have any mic issues so far.
