With Samsung, does FIRMWARE mean O.S.?

Just got this message in Kies, and not sure if it means it will be the ICS OS update, or is this just a firmware update for the phone?

(I don't want to upgrade to ICS yet)

I have been running the latest 2.3.6 for a few months.

Yes, firmware means that there is an update for your phone. Judging by the image, your Note is XEF (Which I believe is from France.) That update is to ICS 4.0.3, 4.0.4 will be coming shortly.
