ROM is Myn's Warm 2.2 RLS4. Only thing I've done I've done is flash one of Jlechner's mods; the "normal taskbar, green widget, super circle battery" combo. Battery widget is Circle Battery Widget. Dock icons are Dreamlyfe. And obviously, LauncherPro.

That's an MIUI rom. He has added one of the custom themes.

You can browse around on the MIUI forum to find the theme, you'll have to email the original provider to request the icons, but good luck. You're better off switching to an MIUI Rom altogether.
