Now I am confused. This log is fubar. The date is WAY off. The recovery type is wrong. The contents don't tell me what I was looking for. There are errors, but I am unsure if they are related. One question, was the last thing you did with the recovery a factory reset or a flash via flashify?

Video of the entire thing, from the phone being off to the end of the wipes, will help me understand EXACTLY what is going on.
 
The log is not telling me that it is wiping anything, let alone loading a script to do so, at least as far as I can tell.

You may wish to also post your issue HERE where hopefully the creator of our lollipop TWRP and others who know more than me can be of more help to you.
 
A shot in the dark..
But have you tried flashing twrp, wipe
Then Boot your ROM and try flashing twrp again

Who knows...
 
Try flashing twrp in fastboot instead of flashify. I've never used flashify so I can't say if that's the problem, but fastboot seems to be the best way to flash twrp.

I know when I first started compiling twrp back when, I had the problem of holding the buttons too long trying to get into twrp and wouldn't be able to get into it. Cwm seems to not care if you hold both buttons too long, but twrp is a little picky. What I do now is power completely down, hold volume down and power, then as soon as I see the factory reset image I let go of power and keep holding volume down until twrp boots.

You probably already know this, but I figured I'd still throw it out there.

I used to think I had problems with the button combo... Turns out it was my case would press both vol up and down if I didn't press it just right.. Felt like such an idiot when I realized:oops:
