As others have noted, I do not mess with the phone itself, other than to add new CM code, which has primarily involved the T9 dialer. Nothing has been changed on the phone itself, in fact we use the stock ril libraries and associated binaries (ril = radio interface layer, it's your cell phone itself).
Returning to stock for a day will help determine if something has been messed up on your custom install. If the stock ROM works without issue, and you still want to run a custom ROM, do a backup of your apps with Titanium Backup, boot into recovery and do a full wipe/format and then reinstall the ROM and gapps.
Although you may not have installed apps that directly affect the phone, many apps gain access to it, and may affect it. You can search through your apps paying attention to the permissions each has. You may find one is causing issues, and you can disable the phone related permissions on that app, or uninstall it to see if it is really the culprit.
I returned to a stock version of the rom from here, so we'll see how things go. I'll leave it as is for a few days and not download any new apps other than updating the ones that came with the rom. I called my phone once from my landline and it took foreverrrrrrrrrrrrrrrr for my cell to even acknowledge the incoming call. 3G speeds are hideous atm.
